NEW STUDENT REGISTRATION

It is time to register your child for the 2025-2026 school year! New Families can register in person during our upcoming Walk-In Registration dates. The schedule will be posted soon—please check back for updates.The information below describes process of enrolling a student for the first time. We look forward to welcoming you to District 163 for the 2025-2026 school year.

New students who are 3 years of age - 5 years of age (if born after September 1st) please contact Algonquin Pre-Kindergarten Center prior to completing the new student registration at (708) 668-9202.

Bring in your required documents by attending Walk-In Registration (dates to be posted soon).

District 163 requires the following enrollment forms and documents. Parents should bring all required documents listed below when registering a New Student.

  • Piece of Mail (if affidavit is required)

Required Student Documents*

All new students must submit the following documents:

  • Illinois State Health Physical dated after September 1, 2024. The physical must be on the Illinois Health Examination Form; no other form will be accepted. The form must have a signature and stamp from the doctor, APN, or clinic with the BMI and Diabetes Risk Assessment done. The health history portion of the form must be completed and signed by a parent/guardian. A copy of the Illinois Health Examination Form is available on our website. Students are NOT registered until we receive a completed physical.

  • Please note that all immunizations must be up-to-date and obtained at the appropriate intervals.

  • Current Eye Exam

  • Current Dental Exam

  • Birth Certificate

  • School Transfer Documents: These documents include Illinois State Transfer Form (Not required if coming from out of state) and Final Report Card for the 2023-2024 School Year.

  • Divorce/Custody/Guardianship Papers/ 906 Placement Form (if applicable)

*Any document that appears to be altered will require further verification.

Parents should bring all required documents listed below to one of the New Student Registration Dates or schedule an appointment to finalize enrollment.

FEES

New student registration requires payment of fees:
Registration -$50*
Student Enrichment - $10
Field Trip - $10

*Parents who receive SNAP/TANF benefits or complete a Fee Waiver Form and are approved will not have to pay the registration fee ($50). All students are required to pay the fees for Student Enrichment ($10)and Field Trips ($10).

Fees are waived for students whose parents are veterans or active-duty military personnel with an income at or below 200% of the federal poverty line.
